How does the program work?

Before we can see you, a physician referral is required. When you begin our program, we will work with you to develop a teaching plan tailored specifically for you, and we will help you set goals that will make living with diabetes more manageable. Feel free to bring a family member with you to your appointments.




It's Your Choice

To best meet your needs, we provide the following teaching options:

Individual Consultation: Individual instruction is available for focused topics such as home blood glucose monitoring, meal planning, carbohydrate counting, insulin use, pump therapy and intensive management. This is a perfect setting for pregnant women and for people with hearing or visual impairments.

Modules: All aspects of diabetes self-management are covered during this group class that consists of four two-hour sessions. For your convenience, morning, afternoon and evening sessions are available.

Support Groups: We offer both morning and evening support groups for adults with diabetes. Meetings are free and are held on a monthly basis. We encourage you to invite family members and friends to join you at the group sessions.




Insurance Coverage

Medicare covers the cost of this program. Most other insurers provide reimbursement, but many plans require authorization before you can be seen. We suggest that you check with your insurance company prior to your first visit. Please bring your insurance card and copayment to your appointment. Bristol Hospital also accepts Medicaid.

To schedule an appointment or for more information, call 860-940-6300, Monday from 9am to 6pm, Tuesday and Thursday from 8:30am to 4:15pm, Wednesday from 8:30am to 3:45pm and Friday, from 8:30am to 4pm.

Our Location

We are conveniently located near the intersection of North Street (Route 6) and North Main Street in Bristol. Our facility offers ample parking just steps from our front door.




Our Team of Experts

Endocrinologist Vijay Joshi, MD, is Board certified in Internal Medicine and Endocrinology, Diabetes and Metabolism. He treats patients with diabetes, thyroid disease, osteoporosis and other conditions associated with hormonal imbalances.

Program coordinator and instructor, Wendy Spurrier APRN, BC, CDE, is a certified diabetes educator and is also Board certified as a clinical specialist in medical-surgical nursing.

Nutrition is the cornerstone of diabetes management. Our registered dietitian and certified diabetes educator, Michele Perriolat, RD, CDE, offers the latest in medical nutrition therapy. She will work with you to provide an individualized nutrition plan and help you incorporate it into your life.

Support group facilitators Karen Cornell, LCSW, and Betsy Gaudian, RN, BC, RD, CDE, offer monthly support groups to allow people with diabetes and their significant others to share concerns and experiences related to living with diabetes.
