Making an appointment

Before you can be seen, a referral from your primary care physician is needed. Dr. Javier and Dr. Mascardo will work closely with your primary care physician throughout the course of your treatment to ensure that you receive the most comprehensive care. To schedule an appointment, please call 860-940-6300, Monday, Tuesday, Wednesday and Friday, from 9 a.m. to 4 p.m.

Payment Information

We participate in most insurance plans. Since many companies require prior authorization before you can be seen, please check with your insurance carrier before your first appointment. You will need to bring your insurance card and co-payment to your appointments and, unless otherwise arranged, payment is expected at the time of each visit. We accept payment in the form of cash or check.
